Office Policies
Cancellation/No Show Policy:
- There is a 24 hour no show/same day cancellation policy for office visits. A $50 charge will be added to your account. Prior to scheduling the next visit, this fee will need to be paid.
- There is a 24 hour cancellation policy on EMG procedures due to the one hour slot that is set aside for your visit. A $100 fee will be placed on your account and will be due prior to the start of your rescheduled EMG.
- There is a 24 hour cancellation/no show fee of $200 for surgeries/procedures.
Refill Policy:
- Refill requests need to be sent through your portal account or call 972-355-9038.
- All controlled substances require a face to face office visit at each refill unless prearranged with your provider.
- Under no circumstances will refill of medications of any kind occur after business hours.
- Routine prescriptions will require follow up visits every three to six months.
